Family medicine - ms3 clerkship:
Site clerkship faculty: Cynthia Ohata, MD
Reporting time: First day 8:30 a.m. The site coordinator will email "Reporting Instructions" to the student a week prior to the start of the rotation.

Family Medicine Clerkship is a six- or eight-week outpatient rotation offered to students of the University of Illinois at Chicago (UIC), Midwestern University Chicago College of Osteopathic Medicine (CCOM) and Rosalind Franklin University Chicago Medical School (RFU/CMS).
Family medicine elective (sub-i):

Reporting time: 7 a.m. The site coordinator will email "Reporting Instructions" to the student a week prior to the start of the rotation.

Orientation and systems training will be done by the Family Medicine senior resident.
Enrollment for this elective opens March 1. All medical students interested in doing the Family Medicine Elective rotation must contact the site coordinator for availability. This elective is a four-week inpatient rotation offered only to MS4. This elective is offered to medical students from affiliated and non-affiliated medical schools. Student must be from an LCME- or AOA-accredited medical school.

All students interested in doing the Sports Medicine Elective rotation must contact the site coordinator for availability. This elective is a four-week rotation offered only to MS4 from affiliated and non-affiliated medical schools. Student must be from an LCME- or AOA-accredited medical school.
Sports Medicine Elective Information
Important information
Rotations will be canceled if required forms are not received and approved by the site coordinator a minimum of four weeks prior to the start of the rotation. Please complete the required forms.
Students from affiliated schools (CCOM, RFU, UIC) must complete:
- Medical Student Information Form and Electronic Medical Record Access Form only.
Students from non-affiliated schools should have their schools send the site coordinator an authorization/approval letter of an outside elective. These students must also complete or provide:
- Medical Student Information Form
- Medical Immunization Form
- TB Questionnaire Form
- Electronic Medical Record Access Form
- Copy of ACLS and BLS certification card
- Academic status documentation
- Malpractice Liability Insurance coverage document
- Proof of flu vaccination for rotations occurring between October and April
