Our hospice care is focused on providing care and comfort for those who have chosen to change their plan of care from aggressive, curative medical treatment to managing pain and symptoms. We focus on helping you live well and with dignity to make every day count.

We take a team approach to hospice care. Our interdisciplinary team of health care professionals, clergy and volunteers brings compassionate pain management, symptom control and spiritual and emotional support to you in the comfort of your own home.

Transitioning to hospice

Patients enter hospice care at various stages of illness. However, those who begin earlier reap the greatest benefits. Care begins with careful assessment by a registered nurse who will arrange for the medical supplies and equipment you will need at home. Your nurse will work with your physician to create a treatment plan, review your current medications and begin coordination of services with other team members. Your nurse will also discuss the “comfort care kit,” – medications to control pain, restlessness, and other symptoms. This lessens the need to go to the pharmacy or wait for deliveries.

Your hospice team will prepare you for what may happen and help you understand how to use community resources to your best advantage.

You are in control

You and your caregiver are in control of your hospice care. Your individual beliefs and values guide how we deliver comfort and care. As a result, you choose which services are appropriate for your needs and can make changes or end hospice care at any time.
