Our specially trained and compassionate professionals take a team approach to hospice care. They make regularly scheduled visits to your home and are on call 24 hours a day, 7 days a week.
Members of your hospice team include:

Patient and caregiver: You and the person you’ve entrusted with your care are the most important members of your hospice team.

Physician: Your personal doctor is a key hospice participant who, along with your hospice physician, makes medical decisions to help maintain your comfort level. Our hospice team provides regular updates to your physician to keep them fully aware of your condition.

Hospice physician: Our physicians are medical experts in symptom management and will supervise your plan of care and maintain close contact with other team members.

Registered nurse: Hospice nurses work closely with physicians and other team members to manage your pain, educate and support caregivers, monitor medications and help make every day count.

Certified home health aide: Our aides assist you with personal care and daily living activities. They also maintain close communication with your nurses, notifying them of any changes in your needs.

Social workers and bereavement counselor: These professionals help meet your emotional needs through all stages of illness. Social workers and grief counselors are also there for loved ones who are caring for you. When necessary, they can recommend other community resources for additional guidance and support.

Chaplain: Our chaplain plays a vital role in hospice care, offering you and your caregivers spiritual support. We will work with your preferred clergy or, if requested, refer you and your caregivers to other faith organizations in the community.

Volunteers: The essence of hospice care is its carefully selected volunteers who can sit with you, help with chores, run errands, make phone calls, and provide small periods of respite for your caregivers. If you or your caregivers simply need to talk, our volunteers are there with a quiet touch and a sympathetic ear.
