  • Adult Day Services: A safe, secure and stimulating environment for adults and older adults who, because of physical or cognitive disabilities, require assistance during the day. Registered nurse, social workers and recreational therapist on staff. Medication monitoring and distribution as well as other nursing care provided. Intergenerational programs, pet and music therapy, exercises, reminiscing and creative arts are some of the programs. Transportation offered within service area. Funding opportunities are available.   • Advocate Health at Home: Our team of professionals offers traditional nursing care and patient education in the comfort of your own home. We understand the complexities of illness and the special needs of patients. That is why we use a primary care approach to teach them how to function best at home. Our trained staff of registered nurses; physical, occupational, and speech therapists; medical social workers; and home health aides work with patients to bring them to an optimal level of independence. Other available services include: infusion therapy and pharmaceuticals, home medical equipment, hospice, home respiratory care services, and chronic disease management.   • Emergency response devices (Lifeline): A security system for those with health risks or concerns who are home alone, allows you to call for help with the push of a button. This system provides 24-hour assistance. Learn more about Lifeline or call 847-296-0737.
  • Expressions: A program for people diagnosed with early stage Alzheimer's disease or related dementia that focuses on different aspects of memory enhancement. The sessions provide a safe, positive, strength-based environment that allows participants to learn, share, and engage together. Components of the program include camaraderie, support, physical activity, mental exercises, creative expression and education regarding memory loss. Activities are designed to encourage members to use their brains in novel ways to enhance memory and foster fun and engagement with others who are experiencing the realities of living with memory loss.   • Home-delivered meals: A support service that provides a nutritious hot meal to homebound individuals who cannot prepare their own meals. Free home assessments available. For more information, call 847-296-0485.
  • Information and Referral: A master-level social worker is available to help you access hospital and community resources throughout the Chicago area. Information is also available on programs across the United States for those caregivers whose relatives are not living close to them. For more information, call 847-723-7770.
  • Outpatient therapy: Specializes in neurologic and medically complex diagnoses. Multi-disciplinary services, including physical, occupational and speech therapy, are offered through a patient-centered treatment program.   • Palliative medicine program: Provides relief of pain, symptoms and stress when facing a serious illness. The palliative medicine team works alongside the primary care physicians and specialists and delivers palliative care simultaneously as the patient undergoes treatment for their disease.
