The Advocate Memory Center is committed to connecting all those affected by Alzheimer's disease and other dementias with the support, resources and education they need.
Advocate community-based programs: Advocate Health Care services – such as home care, adult day services, and home-delivered meals – that support individuals who are living in their own home setting. These services can help individuals to maximize their functioning and remain independent as long as possible.
Support groups: Regularly scheduled meetings at which family and friends caring for a person with dementia can give and receive emotional support, resources, and information. Meetings are facilitated by a Licensed Clinical Social Worker affiliated with the Advocate Memory Center.
