Overview of resident supervision

Supervision is the cornerstone of Practice Based Learning, one of the ACGME’s six competencies. Our training program provides regular supervision to residents at all levels of training as described below:

PGY-1 and PGY-2

All residents begin their training on the inpatient adult psychiatry service with direct supervision. This includes on-site “buddy-call” with a more senior resident throughout the first month.

As residents achieve specific educational milestones they progress to a mixture of Direct and Indirect Supervision. In both scenarios, regular in-depth teaching discussion occurs with their supervising psychiatrist.

By the end of the PGY2 year residents will generally be seeing the majority of their patients independently, with supervision continuing on a regular basis.

Urgent supervision is available at all times to all residents by all inpatient faculty.

PGY-3

During the PGY3 year residents transition to the outpatient clinic. Supervision is structured through a regularly scheduled format each week:

  • In-Room Supervision is schedule for one hour each week and consists of a new patient evaluation performed with an attending supervisor in the room for the entire assessment. Discussion of the case occurs immediately after the patient interview is completed.
  • Case Review occurs at the end of every appointment when a faculty member joins the resident and the patient for a discussion of the assessment and the treatment plan.
  • Case Discussion occurs at the end of every day when each resident meets with a faculty member for 30 minutes to discuss the day’s cases and answer any questions.
  • Psychotherapy Supervision occurs every week for an hour. Residents select a small number psychotherapy patients to discuss in depth with their supervisor. This experience may also include the use of video recorded psychotherapy sessions with the patient’s consent.
  • Chart Review Supervision occurs daily. All progress notes are reviewed by a faculty member, and feedback is provided as indicated to promote resident growth.
  • Urgent supervision is available at all times to all residents by all outpatient faculty.

PGY-4

During the PGY4 year, resident autonomy develops further, and supervision evolves accordingly:

  • Case Review occurs at the end of every appointment when a faculty member joins the resident and the patient for a discussion of the assessment and the treatment plan.
  • Case Discussion occurs every week for one hour and consists of discussion of cases seen and treated.
  • Psychotherapy Supervision occurs every other week for an hour. Residents select a small number psychotherapy patients to discuss in depth with their supervisor.
  • Chart Review Supervision occurs daily. All progress notes are reviewed by a faculty member, and feedback is provided as indicated to promote resident growth.
  • Urgent supervision is available at all times to all residents by all outpatient faculty.
