As part of our weight management services, we offer the evidence-based HMR Plan, which has been offering effective weight loss to patients for over 40 years. The program combines structured diet plans, health education and behavioral change to help participants typically lose between 20 and 100 pounds and gain the skills to live a healthy lifestyle. The program is available in-person and virtually.
Cutting calories to lose weight often sacrifices nutrition, so our program emphasizes complete nutrition at every stage. HMR meal replacements offer easy-to-follow diet plans which temporarily replace the meals you’re currently eating, limiting decisions around food and maximizing weight loss. Meal replacement options include prepared entrees, weight loss shakes, soup, multigrain hot cereal and nutrition bars.
A dedicated team supports you every step of the way. Your HMR coach leads weekly group classes and works with you between classes providing one-on-one support. You’ll also use the HMR app, available on iPhone and Android, to keep you on track.
Rooted in behavioral science, the HMR Plan helps you create new habits around healthier eating, physical activity and building a supportive environment. HMR’s multiphased program guides you from losing weight quickly to sustaining weight loss through self-management.
Using one of HMR Plan’s structured meal replacement plans and receiving weekly support and coaching, you can expect to lose 2-4 pounds per week, although the length of time will vary based on your individual goals.
Data from the HMR Plan consistently shows weight loss as a result of lifestyle changes. It’s well reported that weight loss is associated with favorable changes in risk factors for comorbidities associated with obesity and with decreased medication needs.
Meal replacements help people lose weight quickly, but what happens when it’s time to stop meal replacements and start grocery shopping and cooking for yourself? The HMR Plan provides a personalized transition of reintroducing “outside foods” such as lean proteins and whole grains, while reducing the number of meal replacements.
Even after you reach a healthier weight, you’ll continue attending weekly group sessions focused on strategies to avoid regaining weight. By tackling everyday challenges, such as eating out and social events, you’ll learn how to be successful at self-managing your healthier weight. We recommend several months in maintenance for the best results.
To join the clinic-based program, participants must first complete an intake screening, which is reviewed by our specially trained weight management specialists.
The need for ongoing medical monitoring will be determined from this screening – possibilities range from monitoring blood sugar glucose to medication adjustments.

HMR meals average $14-$17 daily and replace most of your current food costs, so you don’t need to purchase additional meals during the weight loss phase of the program. Once you transition, the amount you spend on HMR meals will decrease as you bring in outside foods.
At your initial screening, the medical team will determine if you require any additional ongoing medical monitoring, which is billable to your insurance.
