- Application submission via Phorcas must include:
- At minimum, 3 Letters of Recommendation
- Transcripts from College of Pharmacy
- Personal Statement/Letter of Intent
- Updated Curriculum Vitae (CV)
- Pre-registered for the ASHP "Match" Program
- Please be sure to address the following questions in your letter of intent:
- What are your long-term career goals?
- What type of practice setting would you like to be in over the next 3-5 years?
- Why are you interested in the PGY-2 Infectious Diseases Residency at Advocate Lutheran General Hospital (ALGH)?
