Home care team

When you choose Advocate Health at Home, you receive compassionate, experienced care from a team that provides nursing care, rehabilitation therapy, patient education and other services in the comfort of your own home.

You and your loved one or caregiver are the most important members of your home care team. We work together to teach you to care for yourself and maximize your function and independence.

Your home care team is tailored to your needs and may include:

• Registered nurses, who coordinate your care plan with your doctor and provide patient education. Nurses are on call 24 hours a day to respond to questions or emergencies.
• Physical therapists help patients restore motion and function, improve mobility, speed recovery, relieve pain, and improve body movement.
• Occupational therapists help patients improve the skills needed for daily living, including learning to use adaptive equipment.
• Speech language therapists work with patients to improve speech and swallowing issues.
• Social workers provide support to patients and families, helping connect them with community resources such as transportation, meal delivery, insurance needs and more.
• Certified home health aides offer personal care assistance, including help with bathing, dressing, exercising and other activities related to personal care.
