Memory center evaluation process

What you can expect

The evaluation is comprehensive and will include some or all of the following components:

  • The physician will take a detailed history and perform an examination of the patient’s thinking abilities and neurological functioning.
  • Laboratory and brain imaging studies may be recommended.
  • A neuropsychological consultation may be requested for a more detailed evaluation of memory and other thinking functions.
  • Once all results are available, a meeting will be held with the patient, family and care team to develop a comprehensive treatment plan.
  • Our social worker is available to provide counseling, support, education, and resources to patients and care partners.
  • If requested, all reports will be shared with the patient, their family and the patient’s physicians.

Additional information

Prior to your first appointment: Please contact the office of your referring physician to request that all medical records associated with this referral are sent to the Advocate Memory Center in advance of your visit. Please include any lab work, brain imaging or neuropsychological testing results. Information should be mailed or faxed to the address below. Cancellations: Please contact us as soon as possible, and no less than 24 hours prior to your appointment if you must cancel or reschedule. Appointments cancelled less that 24 hours in advance may result in a charge.

On the day of your appointment: Please arrive 15 minutes prior to your appointment time. Valet parking ($3.00) and guest service volunteers are available; free parking is an option at the main garage and west garage. Take the P elevators to the 5th floor of the Parkside Center. Please be sure to bring your insurance card(s) and picture ID to your appointment.
