What you do each day can make a tremendous difference in how much diabetes impacts your life. As a result, our home health specialists are focused on teaching you how to take charge of your disease. We work one-on-one with you to develop an easy-to-follow program, monitor your progress, and answer any questions.

Below are a few ways Advocate Health at Home can help you manage your diabetes if your condition warrants:

  • Help with the transition home after a hospital stay
  • Monitor your condition
  • Teach you how to measure and administer your insulin and other medications
  • Instruct you on how to check your blood sugar levels and what to do if they are abnormal
  • Teach you how to identify warning signs of potential complications and take action to prevent a hospital readmission or emergency department visit
  • Provide dietary counseling to keep your diabetes under control
  • Teach you the importance of foot care and how to prevent and identify foot wounds
  • Provide wound care if necessary
  • Administer IV steroids and other therapies
  • Connect you with services for help in quitting smoking
  • Help you create an advance care plan so your health care wishes are spelled out for your family and physicians
  • Educate you on transitioning to hospice care later in the disease process if you seek comfort instead of curative treatment
