In December 2022, all Advocate Health hospitals in Illinois posted 2022 Community Health Needs Assessment (CHNA) Reports. These reports describe each hospital’s defined community including a demographic profile, SocioNeeds indices and health profiles derived from both primary and secondary data sources. Each hospital’s specific assessment methodology, partners, need identification and prioritization strategy are also outlined. The reports present preliminary health improvement plans regarding how the selected priorities will be addressed. The hospitals’ 2022 CHNA reports are provided below for members of the community to view, print, and read. There is also an option to provide feedback via a link in each report, if desired, in compliance with the Patient Protection and Affordable Care Act (ACA).
All Advocate Health hospitals in Illinois have an established Community Health Council that is composed of local community stakeholders and internal Advocate leaders. All hospitals participated in collaborative assessment processes, involving public health departments, other health care providers and community-based organizations within the counties in which they serve. Supplemental reports conducted by local organizations or health departments can found beside the hospital report.
Advocate Illinois hospitals developed 2023-2025 implementation plans, obtained governance approval and posted their plans prior to May 12, 2023. These plans outline strategies to address the key needs each hospital selected to address through their hospital CHNA process. In addition to the Advocate Health Illinois hospital 2022 CHNA reports and Community Health Implementation Strategy plans, the CHNA Reports and Community Health Implementation Strategy Plans for the previous cycles remain on this page, accessible for viewing by the community. All hospital CHNA reports reflect a 3-year assessment cycle. Please note that the 2022 CHNA report reflects the 2020-2022 assessment cycle. This dual cycle posting meets the requirements of the ACA.
