Release of information

How do I obtain a copy of my Advocate Medical Group (AMG) medical records, bills or radiology films?

  • For copies of your AMG records, you must complete an authorization form. Click here [PDF] to obtain an "Authorization for Release of Patient Health Information" form or call the Health Information Department at 224-225-0888, Option 2 (within Advocate 19-0888) to have one mailed to you.
  • To obtain the "Authorization for Release of Highly Confidential Health Information" form, click here [PDF].
  • For your hospital records, please contact your Advocate Hospital's Health Information department. 
  • For copies of bills, please call AMG Central Billing at 847-390-5900
  • For copies of Radiology images, please call the AMG site location of where you had your test.

How would I return the form(s) to you to process my record request?

  • You may fax back the completed form to 224-225-0850.
  • You may e-mail the completed form to AMG-ROI@advocatehealth.com.
  • You may mail or bring the completed authorization to

Is there a fee for copying records?

  • Advocate Medical Group uses the vendor Midwest Medical Record Association (MMRA) for record copying. MMRA fees are in compliance with the copying fees are required under 735ILCS5/8-2006
  • If records are going directly to a physician's office there is no fee charged to the patient.
  • There is a fee for copying records that is regulated by the State of Illinois. The fee is updated on a yearly basis by the Office of the Comptroller. You may click here [PDF] for the medical records copy fee schedule.  

When will my records be ready? How will I receive them?

  • Your records will be ready in 7-10 business days following invoice payment. To check on your request status, please contact our Release of Information Vendor, MMRA, at 224-225-0888, Option 2.
  • If you wish to pick up your records at the Oak Brook office, please indicate that by writing 'will pick up". Photo identification is required before we can release your records.

Will you ever fax my medical records?

  • Only in the case of an emergency or immediate appointment, when we will not be able to have records delivered on time through the mail.

How will I know how much my records will cost?

  • A MMRA invoice will be sent to your home requesting payment prior to record release if you wish to have it mailed.
  • We will not know how much your record set will cost to produce. However, we will release a set unless you specify on your form that you want to be aware of the cost prior to mailing records. Call 224-225-0888 Option 2 for the cost after the 7-10 day waiting period regarding status.
