If you or a family member were born at an Advocate hospital and need to obtain a copy of a birth certificate, please contact the Illinois Department of Public Health at 217-782-6553 or use the instructions on their website.

For copies of medical records, please contact Health Information Management at 847-990-5250. You will be required to complete and sign a release form, which is available in English and Spanish.

Public door to door small van transportation may be available. Please call PACE at 847-223-2245 or contact your local town hall for information.
