Our distinctive program offers a safe, secure and stimulating environment for older adults who, because of physical or cognitive concerns, may require assistance throughout the day. Our center is open from 7 a.m. until 5:30 p.m. weekdays and features an activity/dining area, living room, rest area and outdoor activity space. A hot meal, planned by a nutritionist, is served at noon in addition to morning and afternoon snacks.

Based on availability, a bus equipped with a lift can provide transportation to the facility.

Our unique medical model includes a team of nurses, NCTs and social workers on site throughout to monitor the healthcare needs of the individual.

Specialized health care services include:

  • Medication administration as prescribed
  • Individualized care plans
  • Tube feeding
  • Wound care
  • Diabetic care
  • Respiratory care
  • Physical, occupational, and speech-language therapy on site, with doctor’s order
  • Experienced registered nurses provide health monitoring and assistance
  • Accommodation for special diets supervised by Registered Dietitian

Transportation

Wheelchair-accessible transportation is available for adults between Advocate Lutheran General Adult Day Services and their home. This transportation is available to adults in the program both morning and at the end of the day, or both.

Program admissions

  • Call for a guided tour of the facility. Ours is a coded entrance so we will facilitate your arrival and a staff member will show you around and explain our various activities and provide you with a folder of information upcoming activities, policies, fees and other information.
  • An assessment appointment will be scheduled when the older adult and family will join the nurse, site manager and recreation staff to learn more about the center. Staff members will get to know the older adult and assess cognitive status, social needs and medical condition to make for an easy adjustment to our program. An attendance schedule will be made.
  • The applicant must provide documentation of negative TB testing, either by a chest X-ray report done within the last year or by skin testing done at the Cook County TB Clinic or a medical facility.
  • An assessment fee may apply

Program benefits

The Adult Day Center operated by Older Adult Services is designed to help families care for older adults with physical and mental frailties. Our program offers many services that benefit both the program participant and the caregiver

Benefits include:

  • Supervision & Companionship. Participants in the program make new friends and become part of a peer group at the center, and our outreach programs help to keep participants in touch with the larger community. Intergenerational activities are often planned with local children’s day care centers and schools to foster friendships and greater understanding between the generations.
  • A Caring Team. A registered nurse, clinical counselor and activity staff ensure that your loved ones medical, physical and social needs are met.
  • Programming for Special Needs. Older adults with Alzheimer’s disease or related dementia receive personal attention, constant guidance and reassurance can make the difference. 
  • Care for the Caregiver. We give caregivers the freedom to go to work or handle personal business during the day with the peace of mind that their loved one is receiving stimulation and attentive care. Monthly support groups provide support as caregivers to share their feelings and experiences with peers.
  • Stimulation & Engagement. Daytime supervision and a variety of activities and services are provided in a secure and stimulating environment. Individual enjoy hobbies including games, creative writing, arts, discussion groups, cooking, group fitness, community service projects and more. Excursions to destinations like local restaurants, the Chicago Botanical Gardens and bowling facilities provide a fun change of peace. 
  • Recognized Leadership. Our first center opened in 1980. Ever since, we have been a model for similar programs throughout the United States – and a pioneer in adult day services in Cook County.
